I use a modified stoplight system: green for good, red for a hard stop, and yellow for a scene break while still wanting to continue play (IE gear adjustments, check-in’s, water breaks etc). Standard non-verbal safeword is a double tap on any available surface or body part a la MMA Personal signs of discomfort include episodes of mutism and physically backtracking
